• There is no fee to become a member of Apple Street, Inc.. However, you will be responsible for your business cards and other printed marketing materials.

• Submitting an online application to join Apple Street, Inc. (ASI) is fast and easy. If you have all required information on hand (please see list of required information below), most applications take less than thirty minutes.

• By clicking “I Agree” at the end of the application process, you affirmatively consent to receive from ASI documentation in electronic form that ASI may legally be required to provide you in writing.

• Your application will not be submitted to ASI for consideration until you click the “I Agree” button at the end of the application process. You can “Exit” the application process at any time to stop the application process. You are not authorized to sell any product, earn any commission on any product, or solicit other individuals to join ASI until:

1. Your application is accepted by ASI. You will be notified by email when your application is accepted.

2. You complete the ASI contract and fax it back to our company.

Who may apply

• You must be at least 18 years of ages

• You must have a social security number and allow to work the United States under applicable law.

• All information must be completed by the applicant. No other person can complete the information on behalf of the applicant.

Information you will need in order to complete the online application

• Prior to beginning this online application, please make sure you have the following information available:

1. Your Social Security Number - By Federal Law, ASI must maintain social security numbers for persons to whom ASI makes disbursement. The number you provide must correspond to your legal last name. Solely, Company name is not allowed.

2. Employment History - You have to be able to provide a minimum of 2 years of employment history.

You are now ready to proceed with the Application Process.

The applicant agrees that Apple Street, Inc. (ASI) shall have the right to run credit, employment or other financial background investigations on the Applicant at any time ASI deem useful, whether such investigations are conducted by ASI or an outside service or third party. The applicant consents to such investigations and consents to the disclosures of any person or entity to ASI of any financial background and employment conducted by ASI or by an outside service or third party.

Under penalty of perjury, I hereby certify that all the foregoing information provided in this employment application is true and correct to the best of my knowledge and I understand that any false representation, misrepresentation or material omission in this application, no matter when discovered, may serve as a basis for termination by the company.
